OUEvents Accessibility Request Form – 2026

Use this form to request an accessibility accommodation for each of the 2026 OUEvents you plan to attend.

We are committed to providing accessible experiences and complying with accessibility legislation. Timely submission of accessibility requests is essential for allowing us to adequately plan and coordinate your accommodation. Once we receive your form submission and confirm your accessibility needs, we will make every reasonable effort to arrange for the accommodation in a timely manner at no additional cost.

While we will make every reasonable effort to support requests received close to an event, accommodations that require advance planning are not guaranteed.

The personal information collected in the OUEvents Accessibility Request Form will be used only to consult on and fulfill your request. This information will be retained in accordance with the OUAC’s Data Retention and Disposal Policy.
